Health Checks - Chronic Disease Management
Early intervention health assessment: Early intervention can help identify children who are at risk of developing mental illness, behavioral issues, or developmental disturbances. It can also help identify children with disabilities or learning support needs
40-45 health assessment: This assessment can help determine your risk of developing diabetes and other health issues. It can include: Blood sugar: A blood test to check your blood sugar levels:
Lifestyle advice: Practical advice on lifestyle changes to reduce your risk
Other tests: Other tests that may be included are a cervical screen, lipids, and fasting BSL/HBA1c%
Risk assessment: A calculation of your risk of diabetes using the Australian type 2 diabetes risk assessment tool
Other factors: An assessment of other factors that may influence your health, such as smoking, nutrition, alcohol, and physical activity
A health assessment can help identify risk factors that may require further health management. It can also help you create a plan to manage your risk factors.
45–49 health assessment: This assessment includes measuring weight and height, blood pressure, and cholesterol. It also assesses the need for a Pap test, mammogram, vaccinations, testicular examination, and prostate assessment. The assessment also calculates the risk of diabetes, fracture related to osteoporosis, and cardiovascular risk.
Indigenous health assessment: An Aboriginal and Torres Strait Islander (ATSI) health assessment is a yearly health check for First Nations people that aims to identify early detection and treatment of common conditions that affect First Nations people, such as diabetes and heart disease.
70+ health check: This health check includes a review of general health, eyesight, hearing, blood tests, social and emotional wellbeing, medication, and immunization status.
Skin check: A doctor checks the entire body for skin cancers or suspicious lesions.
Menal health: A mental health check can help diagnose mental health conditions such as anxiety, depression, schizophrenia, and eating disorders. Early intervention can play a key role in preventing symptoms from worsening. Some signs of a mental health issue include:
Feeling worried, depressed, guilty, or worthless
Changes in sleep, weight, personal hygiene, or activity
Significant mood swings
Feeling tense and anxious all the time
STI: If you are sexually active, you should get tested for chlamydia every year between the ages of 15 and 29, using a simple urine test. Chlamydia is very common and does not always show symptoms.
If you are at risk of other sexually transmissible infections, such as HIV/AIDS or herpes, ask your doctor about further testing.
Lifestyle counseling: This can be tailored to an individual's health needs and preferences. It can help prevent chronic diseases, improve overall health, and reduce healthcare costs.
Health promotion: This can include diet, nutrition, smoking/alcohol reduction and physical activity
